A quick search, using Duck Duck Go, brought up more than a half dozen available sources, mostly free.
The files tend to be .pdf or .docx.
pdf files can't be opened in Pages, but they can be opened in Preview, copied from there and pasted into Pages.
Pages can open MS Word (.docx) files and translate them into Pages documents. Some reformatting may be needed, depending on whether the author used any Word features not supported in Pages.
It should be relatively easy to reconstruct the form on Pages, given the pdf and docx examples. If you do run into difficulties, let the community know with a reply in this discussion.

Pages does not support form creation, or forms created in MS Word documents. These would be removed from the document on opening in Pages. If the font reference is not installed on the Mac, it will be replaced with Times. If there is enough usability in that eviction notice after opening in Pages, you can tweak to taste, save as a new Template, and then access that Template via My Templates in Pages Template chooser.
If you have access to Word 16.31 or later on the Mac, you can open a PDF in it and have it automatically converted to a Word .docx document. If that preserves forms in Word, then I would recommend using Word, rather than the preceding Pages suggestion.
