How do I request a refund for a subscription (on iOS App store) that was not delivered?

I have purchased a subscription through an app. However, the app is definitely not working as described. I raised two support tickets, the first replied that they'd fixed the problem (they hadn't), the second replied that there was nobody there to help me!
