It sounds like you have enabled AssistiveTouch. To disable, go to:

Settings -> Accessibility -> Touch, then turn off AssistiveTouch.

...adding to the question - when I press the icon, the 3 circles is one of the "choices" I get, and it's labeled "Home". Can't find where to turn this off.
