You may be experiencing Audio Descriptions, though, not VoiceOver (which is a different feature).
Audio descriptions provide an audible narration of important onscreen action and content in movies and TV shows, if available within that stream.
- It is an Accessibility feature that can be turned on|off as default in Settings.
- The setting may be invoked (accidentally) through the Accessibility Shortcut.
- The Audio Description track may be changed as a language track during playback. Audio Description tracks are marked with “ AD” at the end of the track name.
