I want to cancel my appletv but have no idea what the user name and password is. Is there an actual phone number i can call to cancel???

How did you sign up for Apple TV+?

The username is your Apple ID account. It has the form of an email address. The password is the password that goes with that Apple ID account.

After sign-up, you would have received a confirmation email with the subscription details, mailed to the account that matches the Apple ID. During the subscription, after the trial period, you would receive invoices by email for the monthly fees, again with the account details. Do you have emails from Apple in your mailbox?

Do you have any Apple devices? Those would be registered with an Apple ID account.

Even if you call, then you still need your Apple ID account. Nothing can be canceled without account verification, else you could modify someone else’s account!
