I don't have the "never" option in my auto-lock settings - max time is 5 minutes. I read somewhere that this is because I have some mgt system on it (e.g. from an org/employer), but it's my personal phone.

ah, that would probably be it, then. I connect to my current employer's email (Exchange). I'll experiment to see if I get the option back if I remove it.

Even if you don’t have a management profile installed, your employer/school can enforce policies on the device if you have set up an Exchange account on it.

I should have been more clear - I don't have MDM installed, it's a personal device that I haven't gone near an organisations MDM with (so I really really shouldn't be part of any MDM system).
I already checked in Settings > General. Oddly, though, I don't have a section under there called "Profiles & Device Management".
If you don't have that option, it means that you don't have any profile on your device.
Coming back to your problem (auto-lock) did you enabled Screen Time restrictions on your device? Check:
Settings>Screentime>Content & Privacy Restrictions... should be this case, disable restrictions, restart your iPhone, then test it again.
