Time limit reached but NO limits are set and not during downtime

My ipad starts to manifest strange behavior this evening saying time limit has reached:

1. I confirm that there is NO app limits by goto screen time -> app limits
2. I confirm that this is not during the downtime I set to prevent apps
3. Restart ipad does not solve the problem: it actually has another issue I noticed: the apps are able to open after immediate start but I assume the screen time kicks in and disables them after a delay
4. The IOS version is 15.5

This is so strange because it was working for me for a week but today I pick up my ipad and it is not working
