I assume you are asking how to do this from iCloud.com.

  1. Log into icloud.com using your iCloud username.
  2. Click "Find iPhone." I new screen will open that shows a map with your devices.
  3. At the top center of the screen it will say "All Devices" Click on it.
  4. After you click on "All Devices" you will see a list of (surprise) all of your devices. In this list, select the device you want to delete.
  5. The map will zoom in on the device, and a small window with the device options will appear over the map in the upper left corner.
  6. One of the options in the small window is "Delete."

Best of luck,

Mark
