Apple charges $199 plus applicable sales tax for any newer MacBook Pro since after the Unibody models. They will most likely perform it as a complete top case swap including the keyboard, trackpad, etc. since the batteries may be glued in. However, some of the newer ones supposedly have separate removable batteries that aren't glued in.

The quality of the batteries that Apple uses are the same as Apple has its contract manufacturers install at the factory, from top-tier manufactures like Simplo or Dynapack.

The prices may vary when performed by Apple's network of repair shops (Apple Authorized Service Providers) although they tend to be about the same price as Apple. They are allowed to charge other fees such as a diagnostic fee, although I suppose most don't to be competitive on price. I've seen where some charge a mandatory diagnostic fee, but will apply all or part of it to the cost of the service.
