My Apple Watch doesn't have an option in settings under display and settings for an "always on option"

How can I fix this? Is my Apple Watch to old? I need it to stay on as I work in the medical field and need it on for at least 20 seconds to take patient heart beats.

Settings/display&brightness/wake duration/wake for 70 seconds.

When you raise your wrist and the watch face lights up tap on the face and the 70 seconds will start.
