Desktop and Finder folders don't update right away when items are added/moved.

I changed my video card recently, which might be part of the problem. If I drag a file to the desktop, it sometimes takes over a minute to show up. If I open a jpeg for editing from a finder window in icon mode, then edit and save it, the icon doesn't change until minutes later. If I save attachments from apple mail to the desktop, they don't show up right away. If I open several image files from a Finder window in icon mode, and delete some of them in Preview, when I close Preview the icons for the deleted files still show in the Finder window. If I open one of the "deleted" files it is actually an image file from farther down the list in the Finder window. It seems like changes to Finder windows don't update right away. I was using an Nvidia graphics card until Apple recently killed the drivers, I haven't had any other issues with my setup.

I haven't extensively tested yet, but apparently safe mode didn't help. Another symptom was if I emptied the trash I would hear the crumpling sound but the icon shows a full can for about another minute. If I put something in the trash, then empty it while in safe mode it empties immediately. If I do the same in normal mode after the safe mode reboot, it still doesn't update the icon immediately.

I ran Drive DX, thank you for that. No problems with any of my drives, even with self-test. I downloaded EtreCheckPro (still free) and ran it. Only major problem was outdated hardware (2009 MacPro, duh), and some minor issues including multiple anti virus programs that had been installed over the years, as well as Nvidia and other useless fragments. I went through the list, then went through my applications folder, and un-installed anything I wasn't using. After rebooting the problem seems to be gone, I will know more after I've used it for a while. I do run Intego anti-virus along side the Apple stuff. I occasionally run Clean My Mac 3 to clean up incomplete uninstalls. Do you think I should remove Clean My Mac since EtreCheckPro set a minor alert against multiple antivirus apps? I don't know what was actually causing the problems, but EtreCheckPro helped me find some things I might not have looked at, and that might have solved it. Thanks again.

Thank you very much for your help. Left Intego antivirus installed, uninstalled everything else, my problems seemed to have gone away. Even with CleanMyMac there were fragments left behind that I wouldn't have found except for EtreCheckPro, thanks again for telling me about it. Next step is to take my 2009 Mac Pro from High Sierra to Monterey, pray for me...
