So, I'm hearing that it can periodically work and then not work? I have latest versions of iTunes on my PC and iOS on my iPhone 12. I enabled "Use Listening History" in the Music settings of my phone and the play counts updated on iTunes on my PC. It only worked once though. Any suggestions? Thanks.

We have a few questions, to further narrow down what you're seeing and hopefully offer some more targeted troubleshooting. When you notice the play count is not being updated, is that for a specific song/album? Is that happening solely when playing the music on your iPhone or also when playing via your computer? Are you playing it via a playlist or straight out of your library? Is the music downloaded to your device?
Also, if you haven't already, consider quitting the iTunes app on your computer and restarting both your iPhone and computer, just to refresh the software and syncs. Restart your iPhone

Playing songs on my computer via iTunes always updates play counts. No problems there. Just songs played on my iPhone. All my music is downloaded to my iPhone. Play counts don't update whether playing songs straight out of my library or from a playlist from my iPhone.
Yesterday, the local Apple Store told me to enable "Use Listening History" in my iPhone Music settings. Then I played some songs on my iPhone away from home. When I synced my iPhone to the PC with iTunes, the play counts updated. I thought I had solved the problem!! However, that was the only time. Additional plays on my iPhone did not update the play count on iTunes no matter how many times restarted both my iPhone and the PC with iTunes.
