My iPhone is connected to Wi-Fi at home, but as soon as I leave it disconnects

this is entirely normal? Your wifi network is only active around your house? When you have a cellular plan It switches to cellular when leaving home.

When cellular not works see this link If you see No Service or Searching on your iPhone or iPad - Apple Support
