12 mini lens cracked

my iphone 12 mini had the outer glass break (without dropping) June 13/22, had it replaced. the glass broke again July 6/22 (without dropping), and now July 28/22 i have noticed a crack on one of the lenses again. is there a recall or defect that anyone know about because i cannot find any information and the third repair isn't covered under apple care. the first two repairs were $129 + tax
