Inaccurate battery percentages

My battery will shown 100% but a minute after me using it, it would die and if I try turning it on it Would turn on but be on like 12% and than it would die immediately but if I put it on charging after it died it would turn on and instantly be on 100% but when I take it off of charging the same thing happens it dies after like a minute of me using it.
