Password leak
I have a lot of password recommendations in my iPhone telling me that various password has been leaked, among them my Apple ID. I find this to be highly unlikely, when was Apple hacked and passwords leaked?

This is a warning message for you because you may have used a weak password which is quite likely to be guessed by anyone. Hence the keychain is prompting you.
If I were you I would immediately change the password. --> If you think your Apple ID has been compromised
