how to check if iphone been hacked

hi,

my iPhone XR keeps freezing and I had to reset it a few times. Since yesterday I'm receiving an insisting message asking me to log in to update apple ID settings while I am already login. I reset, and erased all content and setting last night. I'm surprised to have the same message showing again. please advise.
