my iPhone 12 Pro has an app that was added called “Burner”. It was added a couple days ago. I see it in settings - purchases. Cost is zero, but,

OK, that is a legitimate app, and can only be installed from the App Store. Do you have Family Sharing, or do you share any Apple ID with anyone? If they installed the app it will also appear on your phone.

No one has hacked your phone, unless you jailbroke it.
Can you post a screen shot of the icon? There are several apps with that name.

Indeed … the most common “compromises” are made by people we know who have access to our unlocked devices.

If you mean different individuals with different devices all using a common AppleID …
… that’s REALLY BAD … and something you’ll want to resolve in the fairly near term.
