Data leak? Apple at fault?

I received a notification regarding password data leak this evening. All 100+ of my passwords have been leaked, majority of passwords are different not repeats. Leaks include username and passwords for my mums and other family members accounts that are stored on my phone. Passwords include new things from the past couple days plus sensitive nhs work passwords, uni passwords all sorts. Does anyone know if Apple has had any breaches as of late as I don’t understand how every single password that’s stored on my device & iCloud has been leaked?
