How do I restore the Notes icon to my iPhone? The notes are there but I have to access them through the App store.

If you can access them through the App Store (i.e., if the App Store shows "Open" for the Notes app), that indicates that the app is still on your phone.
Apps can be removed from the home screen without being deleted. Go to the App Library (swipe left past your last home screen). From there you can touch and hold an app and slide it left back to the home screen. There is a search box near the top of the App Library.
