I blocked a number on my phone. He text me and it didn’t show on my phone, but came up on my watch. My question is, will the message come up as delivered to the sender or remain unsent?

Since the message appeared on your Apple Watch, most likely it will show as delivered.
To prevent that from happening again, check this setting:
This should stop any messages that do not go to your iPhone from coming to your Apple Watch.
