creating forms in Numbers still a mystery
My table is loaded with data, but hitting + only offers "add a sheet."

That being the case, you could use the Provide Numbers Feedback menu item in Numbers (Mac version) to request that 'Forms' be added to the Mac version of Numbers.
Rationale behind adding Forms to the iOS version appears to be concerned with the portability of and lack of screen space and keyboard of iOS devices compared with the desktop and laptop devices.
