For the PUK code contact your carrier, only they can provide the code or issue a new sim card.
What iPhone are you referring to?
How do you not know if it has a SIM card or not.
If its requesting a PUK code it should have a sim card as its the SIM card that gets locked with the PUK code.
Can you not check the SIM card tray?
Does the iPhone show a "No sim" error at the top or not?
