TMobile gave me a physical SIM card for my mom’s Apple five series watch when I switched her over to our plan from ATT. Do I need to install it?

No, there's no way to install a physical SIM card in an Apple Watch. T-Mobile should have activated the service directly on the watch via a digital sim also known as an eSIM.
