Contact the carrier that the iPhone is locked to and ask that carrier about the requirements to get that iPhone unlocked. iPhones can only be unlocked by carriers that the iPhones are locked to.

An iPhone purchased for full price directly from Apple in the USA will be unlocked out of the box. An iPhone purchased from any other merchant will lock to the first US carrier it is used with, and MUST initially be used with a US carrier. After it has been used with a US carrier that carrier can unlock it; each carrier has their own requirements to unlock, so you must ask that carrier. If you buy a phone that is financed by a carrier, or bought from a carrier, it will be locked. Again, you must ask that carrier to unlock it.
