When using Bluetooth headphones the alarm will go off in the headphones but not the phone and when I don’t have the headphones in it will sound through the phone. Please help!
iPhone 12 Pro Max, iOS 16
Although the related article sways, that the alarm should go off on the phone and the headphones, that does not work on my iPhone 12 either, latest iOS 16.0.3 installed.
If the AirPods are connected, the alarm will be heard over the headphones, if they are disconnected, the external speaker of the iPhone will play the alarm sound.

This only started happening in the last couple weeks, before then it would play the sound from the alarm out of both the headphones and the phone speakers, unless an update changed that, that’s not true.
Did you already force restart your iPhone?

Also make sure the Mute switch is working correctly and the volume is set to your needs.
This might be possible. Maybe you should also report that issue to Apple, using the Feedback Link:
It is supposed to work this way. If connected to Bluetooth headphones or speakers, all sound, including alarms and notifications will be sent to the connected output source.
I did try that but no help.
