AirPods need twisting forwards to get full sound

I have AirPods version 4E71. after the first year I have had to twist the AirPods forwards and upwards to hear the full sound making them very uncomfortable to wear as the shape no longer fits my ear. I have cleaned them as per instructions and had my own ears cleaned but it has made no difference. If I fit them normally the sound is so low I cannot hear. Any suggestions how to resolve this please?
