Multiple watches on one iPhone

I'm conducting a medical study using a standalone watch app. Is it possible to set up 5 watches as family members using one Apple ID, the sign out of that Apple ID on the phone, sign in with another Apple ID on the phone and then set up 5 more watches as family member devices? In summary, can I have 2 separate parent phone accounts each pairing 5 child accounts in order to download the app. The watches would be given to patients to use at home.

I think in such a unique situation the best advice is "Try it" but I think you may need the parent iPhone to remain set up with the parental Apple ID for occasional maintenance. ie

"You'll need to have the role of organiser or parent/guardian to set up an Apple Watch for a family member." in the article below.

They also need to be cellular watches with an eSIM set up to function away from the parental iPhone.

I would also remind you of the Apple caveat -- Apple Watch should not be considered a medical device so be very careful what you want to use them for and obtain the correct FDA or similar approvals for professional use.
