I am trying to extend my Windstream kinetic network to an apple extreme ac device via an ethernet cable. when I go through the setup on my iPhone or MacBook Air, when it gets to the 'other options' area it cannot complete the extension, and only gives an option to create a new network. Is there more that can be done?

It might seem unintuitive, but set up the AirPort Extreme to "create a wireless network" that uses the same wireless network name and same wireless network password as your main router and you are done.

You are likely confused because Apple's "Extend a wireless network" setting on the AirPort Extreme would only be used if the AirPort Extreme was connecting using wireless to another Apple AirPort router.
But, you are connecting the AirPort using Ethernet.....not Wi-Fi....and "Create a wireless network" is the correct (and only) setting to use in this case.
