Keychain will not save updated password (when I choose to Update the Password) in the keychain when changed. I must physically delete the keychain entry (or manually update the new password) in order to store the password change for future logons.
This problem seems to have started with iPadOS 16 update.
Has anyone else experienced this issue?

I don’t recall seeing reports of Keychain issues being linked to the iPadOS16.x update.
As a diagnostic exercise, disable Keychain in iCloud from iPad settings; if prompted, save a local copy of the Keychain as you do so - then re-test adding/changing a Password entry.
Settings > [Your Name / AppleID] > iCloud > [Apps Using iCloud] Show All > Passwords & Keychain > Sync This iPad - set to OFF
