I bought an Apple Watch for my father. He’s in the early stages of dementia. The watch has cellular capability. The problem is he can’t remember anything I teach him, so the thing has to be bulletproof. Anyway, the main problem I’m running into is, I understand. I have to create an Apple ID for him, but do I really have to sign out of my own device in order to do that?

First create his Apple ID in website https://appleid.apple.com
You may find it easier in a computer or ipad. Don't sign-in, look for the create apple ID link which will appear on the top RH corner in computer or ipad Safari views
