iPhone led flash doesn’t flash while on dnd when someone regular call.

We understand that you're notified of FaceTime calls but not audio calls, correct?

Are these contacts that are calling in your Favorites? If a contact in a Favorite, it will bypass Do Not Disturb as outlined here: Use Contacts from the Phone app on iPhone - Apple Support

"Add a Favorite

You can add VIP contacts to your Favorites list for quick dialing.

Select a contact, scroll down, then tap Add to Favorites.

Calls from these contacts bypass Do Not Disturb (see Turn on or schedule a Focus on iPhone)."
