All of a sudden, my iPad, no matter what I’m doing, keeps going to the log off screen. When I hit cancel, it makes me renter my password. This happens over and over. At times it just seems to lock up. It also goes to a black screen and even weirder, takes a screen shot when I press the home button.
I updated to the newest operating system, tried restarting, etc - nothing seems to work.

If restoring it back to factory settings and setting it up as a new device does not work, the issue is hardware related and the iPad needs to be serviced:
