locked iphone se

I gave my 81yr old dad a phone and have him on my account. he entered a passcode wrong so many times it required me to connect to itunes. i followed the restore & update process, now it wants the apple id / icloud info which he has no idea. how do i recover his id/icloud info to get his phone working for him again???
