Apple Watch rash

After 3 years of use, my Apple Watch is now causing a rash directly under the sensor. Nothing has changed about how I use my Apple Watch. I keep it clean and dry, I keep my wrist clean and dry. This has started within the last couple months. I have tried it on the wrist I don’t normally wear it on, and now I have a rash on both wrists. The rash clears up with some days without the watch on, but comes back when I put the watch back on. Has this happened to anyone? Why is it only starting now? I wonder if it’s something to do with it being old??

First, it's probably not something you're doing. I swear, no one understands allergies. People eat shrimp for years and then, BAM, suddenly they need an epipen just to walk into Red Lobster! Who knows?

People have reported allergic reactions to some stuff, like nickel, used in the watch. See "For people who are sensitive to certain materials" here: Wearing your Apple Watch - Apple Support

You should, in order:

Take off the watch (which you did--good!)

See a doctor. A dermatologist, probably.
