Kids screen time turns off completely when he does “one more minute”

Hi, I have set up screen time limits & parental controls on my sons’ phone. We’ve been using it for a couple of years without major issues. I recently got a new phone (parent, same appleid) and re-set up his screen limits/controls as before. He’ll sneak an extra minute on apps with the “one more minute” option, but now when he does this on a couple of apps, it’ll turn off all the parental controls, including allowing in-app purchases, downloads and all the screen time limits are gone.

this is a panic-inducing situation.

I have turned on/off both phones, both are on latest iOS, I have turned off screen time and started parental control set-up from scratch.
