What is this Feedback app showing on my iPhone?

What is this app? I only see it in shortcuts, I don't have it anywhere else. I was not a member of ios beta. Is it possible someone installed a fake malicious app for me?

Unknown app/ virus question I went into settings, and I saw an astronomy app Is this a virus? I also went to a website and I tried to download a picture and it said that 19 viruses had been uploaded to my phone It suggested that I downloaded an app to protect my phone. Does the app take the viruses off immediately has this happened to any of you guys?

The Truth About Virus Pop Ups Those pop up’s are a real SCAM! They are trying to make you instal an app that’s either bad for your phone or can hack your phone for real! If you see any of these popups, just close them. Nothing to worry about!😊
