Your Disney+ subscription as started in the Disney+ app on an Apple device is valid for the Disney+ app on any of your Apple devices. For a new Apple TV box, use Log in with mobile device to copy the subscription details from there.
This subscription through Apple would not be valid for the Disney+ app on third party devices. If you need multi-platform support, then create a Disney+ account directly with Disney (on the web) instead.

If so, and the issue persists, can you provide. a few more details? Which tvOS version do you have installed? What happens when you attempt to access Disney Plus on your Apple TV? Also, do you see any errors or alerts?
