From where did you purchase the power adaptor?
Was it from Apple directly or from an authorized Apple vendor; was it still in it’s original tightly-sealed Apple packaging; and did you break the tamper-evident seals yourself?
Counterfeits can be cosmetically quite convincing … and yet VERY dangerous.

FYI, we’re all other users, not Apple …
… that said, it sounds like you need a replacement adaptor AND Apple would want to be aware of a possible defect.
Since Cspire apparently shunted you off and directed you to the manuacturer; you’ll need to open a ticket thru Apple Support by following link at the top of the page.
I am very disappointed in this. I’m thankful that it didn’t hurt my son nor burn down my house

Yes it was sealed in the original packaging
That’s certainly troubling.
Even a severely mis-wired receptacle (e.g. 240 VAC on a 120 VAC outlet) should not result in such a catastrophic failure.
I trust that Cspire is replacing the unit?
