Make sure to have a good backup of all your data.
Then start over with a clean install of macOS by first erasing the whole physical drive (Intel Mac only) followed by installing macOS & restoring from a backup. Within Disk Utility you may need to click "View" and select "Show All Devices" so that the physical drive appears on the left pane of Disk Utility. If you have an M1/M2 Apple Silicon Mac, then just erase both "Macintosh HD - Data" volumes...the one part of the Container should prompt you to delete the "Volume Group" as well..do so. Leave all other volumes & containers as they are on Apple Silicon as they are required for booting the Apple Silicon Mac even for recovery mode.
