You've already answered your own question.
And what was the result when you followed the instructions on your screen and contacted your carrier to get your PUK (Personal Unlock Key)? You can probably also get it by logging in to your carrier’s user portal on their website.
Ask your carrier to help you unlock your SIM card using the default SIM PIN or PUK code.
