For me, it turned out to be the Assistive Touch feature gone bonkers.

The fix:

Hit the side button three times to bring up the Accessibility Shortcuts menu and deactivate Assistive Touch. It may take a couple of tries but it should fix the problem.

Hi rkonwolf,

Thank you for using the Apple Support Communities! We see you were linked from the article "If your Apple ID is locked or disabled". It seems those steps didn't help, so we'd like to take a deeper look. To clarify, are you seeing the Activation Lock screen on your device? If so, the steps below can help:

How to remove Activation Lock

Remove Activation Lock on the device
Follow the onscreen steps to enter the Apple ID and password you used to set up your device or your device passcode.
Learn how to find your Apple ID or reset your Apple ID password.

Remove Activation Lock on the web if a device is offline
1. Go to www.iCloud.com/find.
2. Sign in with your Apple ID and password.
3. At the top, click All Devices.
4. Select the device that you want to remove from iCloud.
5. Click Remove from Account.

Start a support request
If you need help removing Activation Lock and have proof of purchase documentation, you can start an Activation Lock support request.

If not, can you tell us where you're trying to log in? For example, are you trying to log in to your Apple ID while setting up your device? Are you trying to set up a Mail account?

Can you tell us exactly what the alert says on your screen? This can give us insight into what you're experiencing.

Hope this helps!

Have a great day.

I'm having the same problem. There are white boxes that show up around things on the lock screen. I think Voice Assist was inadvertently turned on, but I can't log in to turn it off. Anything I try to do anything, the voice assist says something super fast and now it's saying "activate default." When I try to put in my passcode, Siri reads out the numbers, but they don't actually register on the screen. I've forced a restart on my phone with no success.
