Remember, you are not inquiring of Apple here. We are end users like you . However, as Niel says, published Apple warranty documents state this:
AppleCare Plus has the same disclaimer.
That statement appears to address warranty service, not pay-for service.Verify with Apple.
I recommend against buying a US phone for use in another country.
1. Yes, but see below.
2. The regular price for that model.
3. Not in the USA models.
4. It's at the discretion of the people at the service center. They likely won't have international iPhone models on hand for replacements, and therefore aren't obligated to honor warranties or AppleCare plans from outside their own country(except when both are in the European Economic Area or Switzerland).

Any unit other than an iPhone 14 originally sold in the United States will have a physical SIM card slot; very old ones use a different card type.
