How to delete an app from my phone and my purchased list?

You can delete your app from your iPhone from the Home Screen or here:
Settings → General → iPhone Storage, tap on the app and select Delete App.
You can't delete the app from the "Purchased" list in the App Store, but you can hide it. Open the App Store, tap your profile picture in the top right corner and select Purchased. Find the app you want to hide and swipe from right to left and select "Hide". The app will be stored in Hidden Purchases.
