iPhone Mini Ringer
I can be sitting right next to my iPhone and a call comes in, only rings once and goes to voicemail. Why?

Call routing including forwarding incoming calls to voicemail are carrier features. As there is no setting to adjust those carrier features on your iPhone, contacting your carrier if this behavior persists would be advised. Please note that weaker cellular signal can also impact call routing and response from your iPhone.
Your iPhone can detect and ignore spam or unknown callers, described in the support article Detect and block spam phone calls. When that is enabled your device should not ring at all and calls will instead be treated as unanswered (because they are) and route to voicemail per your carrier settings if applicable.
