How to know if my iPhone is hacked?

I believe my phone is hacked or at times someone is in my phone

The chances of your phone being "hacked" are remote in the extreme. Not even the FBI could hack an iPhone. What is more likely is you are seeing something that is normal behavior but you do not understand. You need to describe to us what makes you think your device is being used by another person. If anything is compromised, its a lot more likely your ID, not the device itself.

About a Hacked iPhone:

Unfortunately, there is no way to determine if it has been hacked, unless you know that it has been jailbroken. And, the only way for an iPhone to have hacked is if it has been jailbroken. And if you purchased It anew from Apple, then no worries there.


So, what to do...

Resetting to Factory Settings:

If you are so concerned, then reset you iPhone to factory settings, as if you were to sell it. Go here: What to do Before you Sell, Give Away, or Trade in your iPhone, iPad, or iPod touch - Apple Support. Perform as instructed there, including the backup creation and iCloud logout.
