I have made a Shortcut as a demo for you which has 7 seven images.

1st two images: Creating a Shortcut ("Mumbai" is the Title of the Note in the Notes App)

The next two are Testing the Shortcut

The last three images use Siri and the Shortcut named "Append to Note"
