Where do I find my SM-DP+Address and the activation code from my carrier on the iphone

These are eSIM activation details that are typically provided by your carrier in the form of a QR code. While you won't be able to find them anywhere in your iPhone settings for reference, if you have that information from your carrier, you can enter it manually with these steps:

Enter the eSIM information manually if necessary
1. Contact your carrier to get the eSIM details you need to enter.
2. Go to Settings.
3. Tap either Cellular or Mobile Data.
4. Tap Add Cellular Plan (or Add eSIM).
5. Tap Enter Details Manually, at the bottom of your iPhone screen.
You should see a screen with options to enter the SM-DP+ Address and Activation Code from here.
