Hello craigfarq,

We understand that the groups you've created for your iCloud contacts have duplicated on your iPhone. To isolate the issue and help us determine next steps, we'd like to have you see whether the contact groups are duplicated on the iCloud website. Please follow the steps below:

  1. Navigate to iCloud using a browser on your Mac, and sign in with your Apple ID.
  2. Open the Contacts app.

In the left side bar of the Contacts app, check for duplicates of the groups you've created.

If the groups are duplicated on the iCloud website, you'll want to manually delete the duplicates there, using the steps in Create and manage groups of contacts on iCloud.com.

If the groups aren't duplicated on the iCloud website, refreshing your iCloud contacts on the iPhone may help resolve this. To do so, go to Settings > [your name] > iCloud. If you're running iOS 16 or later, tap Show All, then toggle the Contacts switch off then on.

Regards.

Groups will never sync when Mac and iPhone are signed with same Apple ID and password on the same network .The duplicate groups are not shown in Mac , but in iPhone it means the iPhone data base is corrupted , back up your iPhone How to back up your iPhone, iPad, and iPod touch - Apple Support , then erase the device Erase iPhone - Apple Support

Note : contacts always sync with Mac / iPhone but groups never .
