Go to the website of an email provider such as Outlook, Yahoo, Gmail, and click on the "create new email account" there, then follow the steps.
If you want to create an iCloud email, then go to https://appleid.apple.com and click on "Create your Apple ID" from there. Every Apple ID will have an associated iCloud email. Do note this will be entirely separate from the Apple ID you are currently using, and have signed in here with and can never be merged.
